Community health workers take on heart disease in detroit and cleveland

Summary

This study aims to reduce heart-related health problems in Detroit and Cleveland by using community health workers to coach people on managing blood pressure and other risk factors. About 364 adults with high blood pressure or early signs of heart failure will receive support to improve their health. The goal is to prevent heart failure and heart disease before they start.
