First seen Feb 28, 2026 · Last updated May 01, 2026 · Updated 7 times
Summary
This study tests a program called Primary Care Connect (PC2) that helps cancer survivors with heart risks stay connected to their primary care doctor during cancer treatment. About 266 adults with early-stage breast, prostate, bladder, or endometrial cancer who also have high blood pressure, diabetes, or high cholesterol will take part. The goal is to see if the program improves management of these chronic conditions and reduces heart disease risk.
