Summary
This study checks if it's possible to send some hospital patients home early and monitor them remotely using a smartphone app and vital sign tracking. The goal is to see if at least 30% of eligible patients can safely be cared for at home instead of staying in the hospital. Around 306 adults who are stable enough and have a suitable home environment will take part.
