Hospital at home: can remote monitoring safely send patients home sooner?

Summary
This study checks if it's possible to send some hospital patients home early and monitor them remotely using a smartphone app and vital sign checks. About 306 adults who are stable enough will be offered this virtual ward. The goal is to see if at least 30% of eligible patients can safely recover at home instead of staying in the hospital.
