Summary
This study tested a one-year virtual support program for older adults with dementia and their caregivers. Nurses and social workers provided regular check-ins to address patient and caregiver needs, using standard plans for managing symptoms, stress, and medications. The main goal was to see if this support could reduce emergency room visits and improve the safety of medications patients were taking.
