Summary
This study tested whether combining health information technology with practice facilitation (coaching for clinics) could increase how often doctors refer diabetes patients to diabetes self-management education and support (DSMES). The study involved 22 clinics and measured referral and attendance rates. The goal was to find practical ways to help more patients access education to better manage their diabetes.
