Tech and coaching boost diabetes education referrals in small study

Summary
This pilot study tested whether combining health information technology with practice facilitation (coaching for clinic staff) could increase the number of diabetes patients referred to diabetes self-management education and support (DSMES). The study involved 22 clinics and measured referral and attendance rates. The goal was to see if this approach is feasible and effective in helping patients access education to better manage their diabetes.
