Summary

This study tests whether patient- and provider-focused strategies can help more people control their blood pressure. About 540 adults with hypertension from Los Angeles County clinics will take part. Clinics are randomly assigned to receive either provider outreach, patient outreach, or usual care. The goal is to see which approach works best to reduce health disparities.
