Summary
This study is testing a new program called the Next Day Clinic (NDC). It aims to see if sending certain patients from the emergency room to a special high-level clinic the next day, instead of admitting them to the hospital, leads to better care and fewer days in the hospital. The study will involve about 1,080 adults with conditions like pneumonia, heart failure, or infections, who are stable enough for this approach.
