New program aims to ease dementia caregiver burden after nursing home stays

Summary

This study tests a transitional care program for people with Alzheimer's or dementia who are leaving a skilled nursing facility to go home, to assisted living, or to long-term care. The program also supports their caregivers. The goal is to see if the program can reduce caregiver strain and improve patient symptoms like agitation or mood changes. About 360 patients and their caregivers will take part.
