Telemedicine relay aims to keep heart patients home

Summary

This study looks at whether a telemedicine program can help vulnerable heart failure patients who cannot attend regular rehab. About 300 patients, along with their doctors and home caregivers, will use video calls and chat to coordinate care. Researchers will compare hospital readmission rates and quality of life before and after the program starts.
