Summary
This study is testing a comprehensive support program to help people better manage multiple chronic conditions like high blood pressure, diabetes, and kidney disease. It involves giving participants home blood pressure monitors and connecting them with a team including community health workers and pharmacists via a smartphone app. The goal is to see if this extra support helps improve blood pressure, blood sugar, and overall health more effectively than usual care alone.
