Summary

This pilot study tests whether checking blood pressure at home and talking to a pharmacist weekly by phone can help manage uncontrolled hypertension after an emergency department visit. Twenty-four participants will be split into two groups: one receives the home monitoring and telehealth program, the other gets usual care. The goal is to see if this approach is feasible and acceptable for patients.
