Summary

This study looks at whether a program that combines clinic support and digital health coaching can improve diabetes and blood pressure control in low-income patients. About 600 adults with uncontrolled diabetes or high blood pressure will take part. The goal is to see if better access to telehealth can help people manage their chronic conditions more effectively.
