Summary
This study is testing whether a telehealth program helps older patients with serious blood cancers have better conversations with their doctors about future care wishes. It aims to see if these structured video calls reduce patient anxiety and distress, and help more people complete important legal forms for medical decisions. The program is being compared to simply providing educational materials.
