Specialist Team-Up aims to tame heart risks in diabetic patients

Summary

This study is testing whether a special clinic where heart, kidney, and diabetes doctors work together as a team can help people with type 2 diabetes and heart disease. It will compare this team approach to usual primary care to see if it improves blood pressure and cholesterol control over a year. The goal is to see if coordinated care leads to better use of recommended medications and fewer hospital visits.
