Summary

This study tests a program called T1CARE, where a community health navigator helps young adults with type 1 diabetes address social needs like housing, food, and healthcare access. 100 participants will be randomly assigned to get the navigator right away or after a wait. The goal is to see if resolving these social problems reduces diabetes distress and improves overall health.
