Summary
This study tests whether having patients with advanced chronic kidney disease (stages 4 and 5) report their symptoms monthly through an electronic questionnaire, with results shared with their kidney doctors, can improve symptom management and quality of life. The trial will enroll 70 adults and compare this approach to standard care over 12 months. The main goal is to see if this method is practical and acceptable, with early signs of whether it helps.
