Summary

This study tests whether a thorough health assessment by a team of nurses, physiotherapists, and occupational therapists can improve quality of life for people aged 65 and older who are applying for home care services. Participants are randomly assigned to receive either the standard process or the Interprofessional Health Needs Assessment (INHA), which screens for mobility, thinking, mood, nutrition, vision, hearing, and social support. The main goal is to see if the INHA leads to better health-related quality of life after 12 months.
