Summary
This study tests whether a nurse-led support program can improve symptoms, quality of life, and care planning for people with mild cognitive impairment or dementia living at home. About 200 participants and their caregivers will be assigned to either the nurse support program or usual care for 12 months. The goal is to see if this extra support helps address needs and reduces caregiver stress.
