New program aims to keep teens with diabetes from falling through the cracks

Summary

This study tests whether a transition coordinator can help 324 youth aged 17–18 with type 1 diabetes successfully move from pediatric to adult diabetes care. Participants get extra support before, during, and after the switch. Researchers will compare them to a group that didn't have this help, looking at clinic attendance, diabetes control, and quality of life.
