Summary

This study tests a personalized care program for adults 65 and older who are receiving chemotherapy. Researchers will create individual support plans based on each person's health needs and check in weekly for 12 weeks. The goal is to help manage treatment side effects better and reduce emergency room visits and hospital stays.
