Summary
This study aimed to test a new support program for adults hospitalized with heart disease. The program would automatically screen patients for social needs like food or housing insecurity and connect them electronically to local community resources. The goal was to see if addressing these basic life challenges could help patients stay healthier at home and avoid being readmitted to the hospital.
