Summary
This pilot study looks at whether having a family doctor and a pharmacist work together to review medications is practical and helpful for older adults in Switzerland. About 250 people aged 65 and older who take at least five regular medicines and have three or more chronic conditions will take part. The goal is to see if this team-based review can reduce inappropriate medication use and improve patient care.
