Summary
This pilot study explores whether general practitioners (GPs) and pharmacists can work together to review medications for older adults (65+) who take at least five regular drugs and have three or more chronic conditions. The goal is to see if this team-based approach is practical and acceptable in Swiss primary care. About 250 participants will be recruited, and the study will measure how many people join and stay, as well as their satisfaction with the service.
