Summary
This trial tests whether a structured medication review and care coordination (TMPC) can reduce drug-related hospital readmissions in adults aged 70 and older who take five or more regular medications. Participants receive a detailed medication analysis, discussions among hospital and community doctors, and a patient interview. The study compares this approach to usual care to see if it lowers readmissions within 30 days and is cost-effective.
