Summary
This study tested whether a medication coordinator could help patients avoid risky or unnecessary drugs after leaving the hospital. 141 patients who took at least five medicines took part. The coordinator worked with patients to review their medications and create a personal plan. The goal was to see if this approach reduced the use of potentially harmful drugs and improved quality of life.
