Summary
This study tests a program called SHAPE that combines physical activity and nutrition advice for older adults after they leave the hospital for a severe flare-up of chronic lung disease. The goal is to reduce frailty and muscle loss, and improve recovery. About 90 participants will be assigned to either the SHAPE program or usual care, and their progress will be tracked for 4 months.
