Summary

This study will include about 12,000 people with diabetes to see if using a remote patient monitoring system (RPM) helps keep blood sugar levels stable. The system lets doctors check patients' health data from afar as part of their regular care. Researchers will compare blood sugar control before and after the system is used over two years. The goal is to see if the new approach works as well as or better than standard care.
