Summary
This study tests a virtual clinic where pharmacists monitor blood pressure, adjust medications, and provide telehealth support to older adults (65+) with high blood pressure. The goal is to see if better blood pressure control can also help prevent cognitive decline. About 1,000 participants will use home monitors and a smartphone app to track their numbers over 24 months.
