Summary

This small pilot study will test a program where nurses and general practitioners help breast cancer survivors who also have heart disease make healthy lifestyle changes. The program includes exercise, diet changes, quitting smoking, and stress-reducing activities like yoga. The goal is to see if the program is practical and can improve heart health.
