Can a continuous glucose monitor keep heart failure patients out of the hospital?

Summary
This study will test whether using a real-time continuous glucose monitor (CGM) helps manage blood sugar better than standard finger-stick testing in people with type 2 diabetes who are hospitalized for heart failure. Researchers will follow 140 adults for 3 months after discharge to see if the CGM reduces dangerous blood sugar swings and prevents repeat hospital visits. The goal is to improve diabetes control and lower the risk of returning to the hospital for heart failure.
