Summary

This study tests whether five virtual sessions with a trained pharmacist can improve blood sugar, diabetes knowledge, medication adherence, and quality of life in adults with uncontrolled type 2 diabetes. Participants are randomly assigned to receive the tele-education program plus usual care, or usual care alone. The trial will also explore how acceptable and feasible the program is for patients.
