ER pharmacists aim to cut drug errors in new pilot study

Summary

This study tests whether a pharmacist-led program can reduce medication mistakes when patients leave the emergency department. About 82 adults who take multiple medications or receive high-risk drugs will be randomly assigned to usual care or extra pharmacist support. The goal is to see if the program is practical and acceptable, so a larger trial can be planned.
