Summary

This study tests whether a trained volunteer visiting a stroke survivor at home for 8 weeks can improve their ability to cope after hospital discharge. About 840 people who had a stroke and are returning home will either get this extra support or just their usual care. Researchers will check how well participants are coping at 3 and 6 months after leaving the hospital.
