Summary
This study tests a structured transitional care program for people with heart failure. The program includes discharge education, phone follow-ups, and home care support for three months after leaving the hospital. Researchers want to see if it helps reduce depression, improve confidence in managing their health, and increase self-care behaviors compared to standard care. Participants are adults with heart failure and reduced ejection fraction who can communicate in Mandarin, Taiwanese, or Hakka.
