This study tests whether giving stroke patients a personalized diet plan within two days can lower the risk of death within 30 days. About 3,084 adults with moderate to severe stroke will take part. Dietitians will track food intake and adjust nutrition, using supplements or tube feeding if needed, to prevent malnutrition.
