Summary
This study is testing a new digital tool called P3-EX, designed to help doctors create personalized exercise plans for their patients. The researchers want to see if the tool is easy for doctors to use and if patients find it helpful. They will also check if using the tool leads to improvements in patients' health markers like blood pressure, cholesterol, and weight over 12 weeks.
