Summary
This study looks at whether a special heart failure care network can help people who were recently in the hospital for worsening heart failure. About 1000 adults with moderate to severe heart failure will take part at five heart centers in Germany. Half will get extra support from a heart failure network, while the other half receives usual care. The main goal is to see if the network helps lower a key heart stress marker (NT-proBNP) and improves quality of life over six months.
