Summary

This study tests whether a team of healthcare professionals using a special algorithm can lower 'bad' cholesterol (LDL-C) more effectively than standard care in people with heart or blood vessel disease. About 100 adults with high cholesterol and known vascular disease will be randomly assigned to either the team-based approach or usual care. The goal is to see if this organized strategy helps more patients reach healthier cholesterol levels.
