Summary
This study looks at whether having extra appointments with a heart specialist at your local doctor's office can help people at high risk for heart, kidney, metabolic, and lung diseases get better treatment. About 138 adults from a previous study will either get these specialist visits or standard care. The goal is to see if the specialist visits lead to better use of medications to control risks like high blood pressure and cholesterol.
