Summary
This study is testing if a special nurse-led education and support program can help high-risk patients avoid being readmitted to the hospital after hip or knee replacement surgery. It will involve 300 patients who have conditions like heart failure, COPD, or are on dialysis. The goal is to see if this extra guidance leads to fewer return hospital visits and higher patient satisfaction.
