Summary
This study tests whether a nurse-led education program can reduce hospital readmissions for patients undergoing hip or knee replacement who also have conditions like heart failure, COPD, or kidney problems. About 300 participants will either receive the extra nurse support or standard care. The goal is to see if the program helps patients stay healthier after surgery and feel more satisfied with their care.
