Summary
This study tests whether a nurse reaching out after a routine clinic visit can help patients avoid unnecessary trips to the emergency room. About 13,000 adults aged 30 and older with certain health conditions will be randomly assigned to receive a follow-up message or call from a nurse, or to get usual care. The main goal is to see if the nurse outreach reduces ER visits within the next week.
