New program aims to help seniors with multiple diseases navigate life after hospital stay

Summary
This study will create and test a tracking program to help elderly people (60+) who have two or more chronic conditions after they leave the hospital. The goal is to see if this guidance improves how well they follow their own care plans. About 148 participants will be enrolled, and their self-care habits will be measured.
