New program aims to help seniors with multiple illnesses after hospital stay

Summary
This study will create and test a guidance program for people aged 60 and older who have two or more long-term health conditions (multimorbidity) after they leave the hospital. The goal is to see if the program helps them follow their self-care plans better. About 148 participants will be enrolled, and the program will be delivered by phone or with help from a caregiver if needed.
