Summary
This study compares two ways of delivering a health and wellness program to people with chronic conditions like diabetes and high blood pressure. One group gets a kit with a tablet and home monitors for blood pressure and blood sugar; the other gets standard phone calls. The goal is to see if the remote monitoring approach improves patients' confidence, health literacy, and ability to follow medical advice over 16 weeks.
