NCT ID NCT07075978

First seen Jun 27, 2026 · Last updated Jun 27, 2026

Summary

This study compares two ways of delivering a health and wellness program to people with chronic conditions like diabetes and high blood pressure. One group gets a kit with a tablet and home monitors for blood pressure and blood sugar; the other gets standard phone calls. The goal is to see if the remote monitoring approach improves patients' confidence, health literacy, and ability to follow medical advice over 16 weeks.

What this could lead to

If successful, this could show that remote monitoring helps people better manage chronic conditions like diabetes and high blood pressure.
