Summary

This study tests whether using remote monitoring devices and video doctor visits helps people with chronic conditions like diabetes and high blood pressure feel more confident managing their health. About 40 adults will use a tablet or phone to send health data to their care team. The goal is to see if this approach improves self-care and health numbers over 16 weeks.
