Summary
This study aimed to create a communication tool to help people with Alzheimer's or dementia and their caregivers discuss and plan for future medical care. Researchers worked with 38 participants to see if the tool improved their readiness to make advance care plans. The goal was to reduce stress and ensure patients receive the care they want at the end of life.
