Summary
This study is testing an online program called e-ALIGN. It aims to help older adults with mild memory problems or dementia, and their caregivers, work with doctors to safely stop or reduce medications that might be harmful or unnecessary. The program sends educational videos and information through a secure patient portal and offers a visit with a pharmacist. The goal is to make medication plans safer and better match what matters most to patients and their families.
