Summary
This study is testing a new coordinated care program called I-CONNECT for older adults who have depression or bipolar disorder along with at least two chronic physical conditions like diabetes or high blood pressure. Researchers want to see if this program, which provides a personal care coordinator and better teamwork between doctors, improves participants' overall health and quality of life compared to their usual care. The study will also measure if the program makes healthcare easier to manage and reduces emergency room visits.
