New care model aims to untangle healthcare for older adults with depression

Summary

This study is testing a new coordinated care program called I-CONNECT for people aged 65 and older who have depression or bipolar disorder along with at least two other long-term health conditions like diabetes or high blood pressure. Researchers want to see if having a care coordinator help manage appointments, medications, and communication between doctors leads to better overall health, less stress from managing treatments, and fewer emergency room visits. Participants will either receive this new coordinated care or continue with their usual doctor visits.
