Summary
This study tested a computer tool called PREVENT, designed to help homecare nurses decide which patients need their first visit most urgently after leaving the hospital. It involved nearly 2,000 patients with conditions like heart failure and diabetes. The goal was to see if using the tool to schedule visits faster for high-risk patients could prevent them from being readmitted to the hospital.
