Summary

This study looks at whether a personalized home exercise program can improve heart and kidney function in adults aged 18-75 who have both chronic heart failure and kidney problems (cardiorenal syndrome). Sixty participants will either receive standard medication alone or medication plus a 6-month home exercise plan. Researchers will measure changes in exercise ability, kidney function, and heart function to see if exercise is safe and effective for managing this condition.
